Recreate a simple design01.. – Graphic Designer

Need someone to recreate the attached logo, need something very similar or if you can give it a better distressed look that would be great.
I'll need this to be created in Illustrator or Photoshop and will need the source files once the project is completed.
If you could attach a sample of this while applying that would be great!
