Design quote form for moving website – Graphic Designer

We need to add a custom quote form to https://streamlinedsocials.com/index.php/the-top-removals

Exactly the same as the quote form on https://citylinkmovers.co.uk/

As you can see it allows the client to do a full survey of the job including an inventory of what needs moved and address and details like if the property has a lift and on 2ns floor etc.

It is also very user friendly and clean.

I dont know how this was designed – I was hoping it was wordpress so I could just copy and paste but unfortunately not

Please let me know a price and time frame for this aswell as provide some examples or explain how you will achieve the desired outcome – I dont want to hire someone then the form isn't working correctly or theres things missing or its not mobile responisve in some ways etc etc so I need reassured that you can deliver the results.
